FAST TRACKS® are innovative short-term
case management programs designed to enable participants to identify their
personal and family strengths and needs, create an action plan to address
identified issues, and develop skills necessary to advocate for their needs.
Each program is designed to maximize actual learning and skill development in
the participants. The intensive 12 hour model is purposefully focused on
capturing the attention of the learners, teaching skills and competencies needed
to move themselves and their families forward, and providing opportunity for
linkage with community resources.
In each program trained facilitators, experienced in both case management and
family counseling and therapeutic interventions, work to “jump start” family
members toward the motivation to address their own needs and take action to
improve family functionality through a multi-modal learning format including
group discussion, educational instruction and role play.
The scope and intent of the FAST TRACKS®
Short-term Case Management Programs are aimed at doing more than simply
providing information. The FAST TRACKS® programs recognize that families
in crisis are in need of concrete skill-building to be able to put into practice
concepts learned.
FAST TRACKS® programs are designed to be
mobile and can be delivered wherever the need exists including schools, courts,
churches, community centers, etc., as well as individual participants homes.
