Wraparound In-Home Services & Intensive Case Management  

Foothills’ Comprehensive Case Management Services are designed to assess a child, adolescent, adult, geriatric or family’s therapeutic and social service needs and to deliver and coordinate the acquisition and effective utilization of any needed services and supports required. These services are time limited, tailored to individual needs, culturally sensitive and focused on the strengths of the individual and family. For children and adolescents, these services are generally provided so as to prevent them from entering the custody of the State. For those clients already in State’s custody, comprehensive case management services are used to enhance and expedite their successful release from custody. For adults with serious and persistent mental health issues, these services are delivered to maximize successful and ongoing integration into the community and workforce. For geriatrics, these services are designed to support elders and their families who need additional home-based supports to maximize independent functioning and prevent movement into an assisted living facility.

Foothills intensive Wraparound In-Home Services involve the delivery of comprehensive case management services as described above, in addition to providing a wide and individualized range of intensive therapeutic and/or clinical services to address specific behavioral health functioning issues. Therapeutic/clinical services may include individual, group and family counseling and therapy related to a specific behavioral health issue. Wraparound services include coordinating and/or providing a full range of medical, educational, vocational, psychological, psychiatric, community and other specialized services necessary for maximum development and functioning.  Intensive Wraparound In-Home Services encompass other program models we provide such as Continuous Child and Family Treatment (CCFT), Continuous Treatment Team (CTT), and Intensive In-Home Reunification Services for DCS Continuum of Care programs.

Intensive Case Management and Wraparound In-Home services are provided under the supervision and licensure of the Tennessee Department of Mental Health and Developmental Disabilities, and accredited by JCAHO.
