Community-Based Services for the:
Licensing and Accreditation:All Foothills Care, Inc. services & programs are licensed by the Tennessee Department of Mental Health and Developmental Disabilities and/or the Tennessee Department of Children’s Services, and are accredited nationally by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).

Transitional Living ProgramThe primary goal for the Foothills Care Transitional
Living Program is to provide supportive services for young adults aging out of the foster care system to
enable successful independent functioning within the community. It is
anticipated that the program will ultimately and collectively yield an increase
in the number of youth who have resources to meeting their living expenses as a
result of increased job seeking skills and educational/vocational advancement;
have a safe and stable place in which to live as a result of increased life
skills and support in acquiring housing (including financial assistance for
those who actively pursue the accomplishment of their goals); attain
education/vocational goals as a result of linkage to educational and vocational
centers where training can be acquired; have positive personal relationships
with adults and the community as a result of increased social and problem
solving skills and improved understanding of interaction/personality styles;
avoid high risk behaviors as a result of improved knowledge and awareness of
healthy life style choices with respect to sexual activity and substance use;
and, are able to access needed physical and mental health services as a result
of linkage to community resources. |
